Education:
Minimum –   Medical Degree; successful completion and graduation from a Residency Program specific to specialty

Credentials/Licensure:
Minimum –   Licensed to practice medicine in the State of Ohio , including DEA and Controlled Substance licenses; Board Certified or Board Eligible.

Other Knowledge, Skills and Abilities
Minimum:
§         Effective verbal, written, and interpersonal communication skills, including good listening skills, with the ability to comfortably interact with diverse populations.
§         Demonstrated ability to work collaboratively in a team-oriented environment with a strong customer-service orientation.

Physical/Mental
            Minimum – Ability to perform essential functions of clinical practice. 

E.  Primary Care Physicians duties include, but are not limited to:
§         Develops and maintains an active clinical practice in defined specialty, performing all of the recognized clinical duties of a physician and providing appropriate healthcare treatment, including, but not limited to,
  • Examination and evaluation of patients
  • Perform H & Ps
  • Determine nature and extent of disease or injury
  • Prescribing treatment as appropriate
  • Collaborate/consult with specialists as appropriate
§         Follows medical protocols as established by the organization and consistent with reasonable medical practice.
§         Provides objective, accurate, thorough, and timely documentation of patient care and meets organizational charting guidelines.
§         Participates in office/after hours/hospital coverage and rounding arrangements specific to specialty and as identified by organizational policy.
§         Actively participates in quality initiative processes.
§         Maintains current knowledge base in primary care medicine through maintaining board certification status, attending seminars, and conducting journal review.
§         Sees patients in a timely and efficient manner, staying reasonably on time for scheduled appointments throughout the day, unless exceptional circumstances occur.

F.  Additional roles taken by physicians in HealthProviders if certain criteria are met:
§         Interpreting sleep study results:
o       If a Primary Care Physician routinely performs interpretations of sleep data collected from home based sleep studies and is not Board Certified in Sleep Medicine, the following criteria must be met:
·         The physician must attain CME regarding sleep studies.  The physician must attend one class per year.  The class/education must be from an accredited organization.
·         The physician must have a mentor/peer within the system who is Board Certified in Sleep Medicine.  The peer/mentor must review 5% of the Primary Care Physician’s studies per year to ensure accuracy. 
·         The Primary Care Physician can only interpret results from select patient populations.  The patient must be between the ages of 18-65, present with moderate to server Obstructive Sleep Apnea, and present with no other co-morbidity.

G.  Staff Development
§         Develops collaborative working relationships with all Mount Carmel HealthProviders and staff.
§         Demonstrates professionalism in appearance, attitude, and communication.
§         Demonstrates satisfactory performance based on organizational evaluation tools.
§         Participates in and/or performs other responsibilities according to office/organizational needs.
§         Promptly reports high risk issues to the risk management associate or Chief Operating Officer of HealthProviders.
§         Serve as a role model for staff by setting a good example, which includes but is not limited to punctuality, demeanor, communication, customer service, work load, etc.

H.  Administrative Relations and Accountability
§         Adheres to guidelines pertaining to proper coding, charging, and documentation for all services rendered.
§         Serve as a resource for administration in various capacities related to a wide range of needs; meet periodically with administration at their request, either routinely or on an as-needed basis.
§         Learn new technologies and commit to providing leadership and training to the staff as new technologies are implemented.
§         Assist in creating a constructive professional atmosphere for the staff by being a positive influence; actions/comments should enhance morale and staff relations;
§         Demonstrates on-going support and respect for administration in all communications, either privately with staff or in a group setting; never participate in creating an us vrs. them (clinicians vrs. administration) mentality.
§         Address any administrative issues directly and in private with those concerned;
§         Participates fully in risk management and organizational integrity educational activities as identified to meet organizational needs.
§         Responsible for compliance with Organizational Integrity through raising questions and promptly reporting actual or potential wrongdoing.
o       Participates in and coordinates with organizational process to collect and evaluate information about hazards and safety practices that are used to identify safety management issues.  Adheres to established policies and procedures, which may include wearing of personal protective equipment.
o       Participates in and fosters a performance improvement approach that includes both intradepartmental and interdepartmental activities.
